Planning for the future and transitions can be hard! The focus of this workshop is to provide education and strategies to help parents, caregivers and professionals prepare their teenagers and/or young adults to transition into adulthood. Topics that will be covered include:

• Planning for the transition: Process and Tips

• How to build bridges and connect with the community

• Planning around strengths and areas of need to support their success

Attendees will participate in interactive discussions and will receive resources to better understand how they can support the young adult.
